How We Can Help You

A care team will be assembled just for you.  Community Reach Center's staff consists of a wide array of mental health professionals, including adult and child psychiatrists and psychologists, licensed professional therapists, licensed certified addictions counselors, early childhood specialists, nurses, case managers, special education teachers, and much more. Your team will work closely with you to identify treatment goals tailored to meet your specific needs, and, if necessary, connect you with other community resources that may enrich your treatment program. 

 

So what's the next step?

Evaluation

First, you will come to a Community Reach Center outpatient office of your choice for a mental health evaluation.  Each of the Center's five outpatient offices has time set aside each week for initiatial evaluations for new clients.  Please check the First Visit Schedule for initial evaluation information at the office of your choice. You will meet with a therapist to talk about particular issues that are concerning you. Based on this initial evaluation, the therapist will discuss treatment options with you. Then, you will be assigned to a therapist and contacted within 1 week to schedule your first appointment.  Your assigned therapist may not be the same therapist you meet with during your Intake.

Treatment Plan

You and your therapist will work together to develop a Treatment Plan or "road map" as a guide to assist you in reaching your treatment goals. You and your therapist will review your Treatment Plan regularly to ensure that you are receiving the necessary services to support you in your recovery journey.
