What to Expect at Your Initial Evaluation Meeting
  • Your initial evaluation at Community Reach takes place at your first visit. Initial evaluations are available on a first come, first served basis Monday through Friday, 8:00 to 10:00am at 8931 Huron Street, Thornton, CO 80260 or Wednesdays, 8:00-10:00am at 1850 Egbert Street, Ste 200, Brighton, CO 80601.  Community Reach Center's doors open at 8:00am.  Please take this into consideration during periods of cold or wet weather. 

 

  • First, you will check in at the front desk and receive a welcome letter and a brief questionnaire to complete.   Front desk staff will review your questionnaire with you, and will either provide referrals if the services you request are not available or will provide intake paperwork for you to complete.   It is important that you return your paperwork to the front desk staff as soon as you have completed it, as it helps your initial evaluation process go faster. You can also download these forms from this website and fill them out in advance.  If you have questions about the documents, please ask the front desk staff for assistance...they're happy to help you.  This process typically takes 15 to 30 minutes.

 

  • Next you will meet with an Intake Specialist to complete your intake process.  The Intake Specialist will provide information about the initial evaluation process and important paperwork for you.  Then the interview process will begin, which includes questions regarding emotional or behavioral issues that are currently causing you concern.  Based on your interview responses, your Intake Specialist will refer you to the Community Reach Center location, program and therapist that can best meet your needs.  The interview with the Intake Specialist takes approximately 1 hour.

 

  • Average time from arrival to completion of your initial evaluation meeting is 2 to 4 hours.
