What to Expect at Your Pre-Admission and Intake Assessment Meeting
  • Community Reach Center offers same day access (it is first come, first served. We provide same day appointments but may run out of available appointments that day depending on the volume of people) for pre-admission and intake assessments, Monday through Friday, 8:00am - 2:00pm, at 8989 Huron St., Thornton, CO 80260 or Wednesdays, 8:00am - 10:00am at 1850 Egbert St, Ste. 200, Brighton, CO 80601.

  • First, you will check in at the front desk to begin the pre-admission process.  Please bring an ID, insurance information, and financial information with you. If the client who is being assessed for services is under the age of 15 and you have either joint or primary custody, proof of legal guardianship must be presented at your intake assessment. The Pre-admission process is first-come, first-served and usually only takes about 15 to 30 minutes. We encourage new clients to arrive early to check in and complete their intake paperwork. If you already completed your Intake Forms at home, bring them with you and hand to the front desk staff.  Otherwise, front desk staff will provide you with all necessary paperwork for you to complete. If you have questions about the paperwork, please ask the front desk staff for assistance. They're happy to help you. Front desk staff will ask you personal and financial information for your health record.

  • Once Pre-admission is completed, you will be assigned an intake appointment for the same day (based on availability).

  • Finally, you’ll meet with an intake therapist who will also ask you questions regarding emotional or behavioral issues that are currently causing you concern. Based on your responses, you will be connected with the appropriate Community Reach Center location, program and therapist that can best meet your needs. The interview with the intake therapist takes approximately 1 to 1 ½ hours.
