Requesting medical Records
In order to request medical records from Community Reach Center, please complete the Request to Access Protected Health Information (PHI) form below:

AUTHORIZATION TO RELEASE AND EXCHANGE MENTAL HEALTH INFORMATION
Copies of medical records may be released to a third party (someone other than the consumer) upon receipt of a valid Release of Information (ROI) authorized by the consumer or guardian/legal representative. To authorize the release of records to a third party, please complete the form below:

Fees or charges may apply, but we will notify you of any applicable fees before we process your request. Custody/guardianship must be confirmed to establish authority prior to a records release. Please include the necessary legal paperwork when submitting your request. If you are seeking verbal disclosures for care coordination purposes, please contact the treatment team directly or by calling Community Reach Center at 303-853-3500.

INTERNET PRIVACY POLICY

Community Reach Center is a private, non-profit organization dedicated to the provision of quality mental healthcare and integrated physical and mental health services.
We value your online privacy. No upfront information about visitors to this site is gathered, sold or shared. Community Reach Center gathers and maintains only the aggregated number of visitor statistics for the website and information that is provided by the visitor is maintained only for the purposes of the stated activity for which the visitor has given the information, specifically:
Newsletter Subscription: Persons filling out the information for newsletter subscription will receive our Within Reach Newsletter either monthly (electronically) or quarterly (via U.S. Mail), depending on which they choose.. This information resides in a password protected database at Community Reach Center and is not shared with any other organization.
Donors: We request limited information from the user on our donation form which is used to thank the donor (including a confirmation of receipt of the donation) and to contact the donor if there is a problem processing the online donation). Community Reach Center acknowledges its donors’ names in its various print and online publications which are available publically. If you prefer not to be so listed, please indicate this on the donation form. Any other Information about donors is not shared outside of Community Reach Center and is never sold. Donors will be added to our mailing list unless you request that this not be done on the donation form. Donor contact and donation information resides in a password protected database at Community Reach Center and is not shared with any other organization.
Employment, Internship and Volunteer Applicants: Information provided by internship , employment and volunteer applicants is never sold. It is shared only among Community Reach Center employees on a need-to-know basis and as required by law.
Clients: Community Reach Center keeps its clients’ information private and confidential in full compliance with federal Health Insurance Portability and Accountability Act (HIPAA) laws. For more information, please click on HIPAA Privacy Policy.

Advertising
We have no third party advertisers on our website, although the logos and links of our sponsors for fundraising events may be posted in recognition of, and gratitude for their support. These sponsors do not receive any information about website viewers.
