How We Can Help You

Community Reach Center's staff consists of a wide array of mental health professionals, including adult and child psychiatrists and psychologists, licensed professional therapists, licensed certified addictions counselors, early childhood specialists, nurses, case managers, special education teachers and much more. A care team will work closely with you to identify treatment goals tailored to meet your specific needs, and, if necessary, connect you with other community resources that may enrich your treatment program.

Intake Assessment

First, you will come in for an intake assessment. Community Reach Center offers scheduled intake appointments. Assessment services are provided Monday through Friday, at 8989 Huron St., Thornton, CO 80260 or at 1850 Egbert St, Ste. 200, Brighton, CO 80601. Both Thornton and Brighton offer scheduled intakes Monday through Friday. Our office hours for intake are 8am to 5pm. Our last intake begins no later than 3pm.

The first step is to complete our intake forms (Community Reach Center - Intake Forms), once you have submitted your completed forms, we will call to schedule your intake appointment. If you would like to check the status of your paperwork or need to reschedule please call us at 303-853-3445.

Hours and locations are as follows:

Thornton Intake Location: Monday through Friday, 8:00am - 5:00pm, at 8989 Huron St, Thornton, CO 80260

Brighton Intake Location: Monday through Friday, 8:00am - 5:00pm at 1850 E. Egbert St, Ste. 200, Brighton, CO 80601

Broomfield Intake Location: 100 Spader Way, Ste. 267, Broomfield, CO 80020

 

Treatment Plan

You and your therapist will work together to develop a Treatment Plan to assist you in reaching your treatment goals, and will review your Treatment Plan regularly to ensure that you are receiving the necessary services to support you in your treatment goals.
