Each of our outpatient locations provides behavioral health services through virtual and in-person settings, supported by licensed clinicians, medical providers, certified peer specialists, and targeted case management.

If getting access to or completing these forms is not possible, don’t worry! We suggest coming early to your first appointment so you can fill them out when you’re here.

One of our friendly front desk staff will give you a call to set up your first appointment with us. This will help us understand your needs better and support you as you start your journey to recovery. 

Now you are ready to walk in to start services with us! To ensure everything goes smoothly, please arrive early in case you need to fill out paperwork. It’s also helpful if you bring along your photo ID, insurance card, and any medical history. If you’re bringing a minor, please remember to bring proof of guardianship.

When you meet with our staff, they’ll ask you about the emotional or behavioral challenges you’re experiencing. This conversation usually lasts between one hour to 90 minutes. Next, our staff will walk you through the next steps and offer personalized recommendations—this may include programs, services, or tools to support your wellness journey. We’re here to help you find the right path forward, every step of the way.
