How We Can Help You
Community Reach Center is staffed with a wide array of mental health professionals, including adult and child psychiatrists and psychologists, licensed professional therapists, licensed certified addictions counselors, early childhood specialists, nurses, case managers, and much more. A care team will work closely with you to identify treatment goals tailored to meet your specific needs, and, if necessary, connect you with other community resources that may enrich your treatment program.

Intake Assessment
The first step is to complete our intake forms (Community Reach Center - Intake Forms), please send them to Intake@CommunityReachCenter.org or bring them with you to your walk-in intake assessment. If you would like to check the status of your paperwork please call us at 303-853-3445.
Hours and locations are as follows:
Thornton Intake Location: Community Reach Center has transitioned to open access intakes as of August 15th 2022. We are no longer scheduling intakes. You can walk in Monday- Friday between 8a-12p at 8989 Huron St in Thornton.
Brighton Intake Location: open walk-ins only on Wednesdays from 8:00am - 11:00am. - First Come First Served! at 1850 E. Egbert St, Ste. 200, Brighton, CO 80601
Broomfield Intake Location: 100 Spader Way, Ste. 267, Broomfield, CO 80020
Treatment Plan
You and your therapist will work together to develop a Treatment Plan to assist you in reaching your self-defined treatment goals. Together you'll review your Treatment Plan regularly to ensure that you are receiving the necessary services to support you in your treatment goals. If you are currently seeing another therapist in the community and wish to receive medication services through Community Reach Center, we will need to transfer your care to a Community Reach Center therapist to ensure the best quality coordinated care for you possible.

- Intake Forms - Naturally there is paperwork to be completed for your first visit. If it’s more convenient to complete your paperwork from home, you’ll find all of the necessary intake forms here. Then simply hand the forms to the front desk staff when you check in for your intake assessment.
