Treatment Plan

You and your therapist will work together to develop a Treatment Plan to assist you in reaching your treatment goals, and will review your Treatment Plan regularly to ensure that you are receiving the necessary services to support you in your treatment goals.   If you are currently seeing another therapist in the community and wish to receive medication services through Community Reach Center, we will need to transfer your care to a Community Reach Center therapist to ensure the best quality coordinated care for you possible.
