Refer a Patient
Referring your patient to Community Reach Center is the right choice for mental health care. We’re fully committed to providing each person with high-quality personalized care to support their individual needs and treatment goals.
If you are a hospital looking to refer a patient to us, please follow the steps below.
Step 1
Complete the Hospital Navigation Referral Form
To make a referral, please fill out the Hospital Navigation Referral Form.
Step 2
Send the Form
Email the Hospital Navigation Referral Form to hospitalnavigation@communityreachcenter.org or fax the form to 303-280-4002.
Step 3
We’ll Call You
One of our friendly front desk staff will give you a call within 24 hours to process your referral.
If you are unable to fill out the Hospital Navigation Referral Form, please call the Hospital Navigation line at 303-853-3800. Voicemails should include the patient’s name, date of birth, name of hospital, hospital social worker’s name, and return phone number. Please include the admission date and expected discharge date if known.
