Service delivery through Telehealth makes behavioral health care more accessible to you or your loved one during the COVID-19 pandemic.
Welcome to Community Reach Center! Your first session with us is called an Intake assessment. Click here to learn about what will take place at this first meeting. New clients will continue to be offered intake assessments in-person at Community Reach Center’s Thornton and Brighton locations after a screening at the door for flu-like symptoms prior to entrance. Rest assured that staff will employ all recommended social distancing practices to ensure your safety and wellness. At the end of your Intake assessment, your initial treatment plan will be developed and services appropriate for Telehealth will be discussed.
Thornton Intake Location: Monday through Friday, 8:00am - 2:00pm, at 8989 Huron St, Thornton, CO 80260
Brighton Intake Location: Tuesdays, 8:00 - 10:45am at 1850 E. Egbert St, Ste. 200, Brighton, CO 80601
Broomfield Intake Location: Please call 303-280-4119 to be scheduled for an intake at Broomfield or go to our Thornton Intake walk-in location and request Broomfield Office for on-going services.
In preparation for your Intake assessment, please click here to download and complete the Intake forms. However, if you are not able to print these, no worries! Forms in English and Spanish will be available when you arrive.
Please bring an ID and insurance information with you to walk-in intake or - if you prefer - a scheduled intake. If the client who is being assessed for services is under the age of 15 and you have either joint or primary custody, proof of legal guardianship must be presented at your intake assessment.
Please call your therapist or 303-853-3500 to consult on how to “meet” for your next session. Telehealth services are provided via safe and secure internet-based technology, meaning that you can “meet” with your mental health provider without being in the same physical location. To support the health and safety of consumers as well as staff, most consumers will now receive their individual and group therapy services via Telehealth during the COVID-19 pandemic.
